Cancel Membership

If you paid through 3M payroll deduction, your membership is in effect for the current year. This form authorizes the removal of your name from payroll deduction for the following year if it is filled out and submitted before October 31. If it is filled out and submitted after October 31, you will be removed the year after next.

For example, if you request removal on 12/20/2025, your membership will still be in effect in 2026, and you will be removed in 2027. Refunds are not available.
